
Case Study 1: A patient struggling with low mood, anxiety, and social isolation made significant progress through regular support sessions, including walking meetings and community engagement, leading to reduced isolation and improved self-actualisation.
The patient was referred by ANP due to low mood and anxiety issues.
They were struggling with day-to-day life, they isolated themselves and cut off from all social interactions.
Patient was also struggling with UC – benefit requirements * job coaching.
How did you engage with the patient?
Initially, work with this patient was over the telephone on the week-by-week basis.
To build a relationship where they felt safe and supported and they could explore different opportunities. We met f2f every three weeks and looked at different support, we have met at various locations to support them to engage and had walking talking sessions which they really enjoyed.
What suggestions did you make?
I was able to contact the patients work coach and work together to support them.
Referrals to the MHWBC the patient had previously worked with Let’s Talk.
Referrals to right to choose for an ASD and ADHD assessment.
What changes did the patient put in place?
They engaged and stepped out of there comfort zone and times this was exceedingly difficult for them especially attending community groups / social enterprises.
What was the overall outcome?
The patient asked for help, and they have gone with it at challenging times they have pushed on. They are self-actualizing and they see their own potential it may be a slow process, but they are moving at their own pace and the social isolation has reduced.
