Why was the patient referred to you?
Patient was struggling at home with partner who was on End of Life care and they needed support to access other services, they needed a pathway someone to help navigate care and support.
How did you engage with the patient?
Initially, work with this patient was over the telephone then regular home visits were booked in and calls to check on their welfare over the course of a year.
What suggestions did you make?
I was able to make lots of suggestions, but this couple just wanted to feel supported and be able to talk through concerns and know someone was there to listen.
CISS where able to arrange a daytime sit to allow the patient time away from the home knowing that their partner was safe and cared for.
What changes did the patient put in place?
Engaged with services and asked for help.
What was the overall outcome?
The patient did die peacefully at home which was the wishes of both parties. I am still supporting the other patient they have engaged with services and community groups to get the added support.
Help and support with the change in benefits due to changes in circumstances.
The patient will continue to get support to pathway them into services more appropriate to them.
